Hi everyone! Thank you for taking the time to review our policies and turn in the required agreements. Below you'll find two items. One is the envelope of documents we need you to print, sign and return to us. The other is our policies. You do not need to print them off, but please be aware of the policies included.
NOTE: Members under 18 must have a guardian's signature and information, but DO NOT need to turn in a background check form.
